It looks like the Enterprise got Rear-Ended by a Miranda Class, and then tried to flee the scene….by jumping to warp 7. And failed to release the External Inertial Dampeners. (Left the E-Brake on.)
The resultant mess was then buried on a planet, and left with the hopes no one would find it….
