for those who are curious on who the grandma is (spoils)
the grandma is actually a doctor from the “demon” sect who used a martial art that transforms the person to look like another person/thing (or something like that, it’s been a while since i read this)

Grandma probs in a powerful position. Either the head of the family of the Hwaju clinic, or someone else important. Either way, I reckon Granny purposely injured herself to test someone(?), maybe the MC.
Either way, I reckon he’s done himself a solid without realising.
