He’s not allowed to smile because he couldn’t save his Mother when it was impossible for anyone to do so? Stupid. If he was a doctor before he should be all too familiar with this sort of delusions.
Aqua commented a couple of chapters back that he can’t tell himself apart from the doctor anymore but his trauna is manefesting as him now. This makes too much sense because the Doctor is the perfect embodiment of his past mistakes and losses.
