I’m not a doctor, but I do know that…
1. you can’t remove the broken pillar piece, cause he might bleed to death.
2. It looks like it pierced the intestines, so it’s going to smell like sh… I mean, it’s going to get infectious, so it can’t remain untreated for long.
He might be fine for an hour, if not moved and the area of the damaged tissue is cleaned, while the “entry point” is being sealed, so that he won’t bleed out, but there is still the issue of internal bleeding… I guess he’s dead. 🙂 (of course he isn’t nor will he die)
