I think the toll of the hourglass is fast aging rather than just getting tired. And im not sure but its like the user of the hourglass is paying by being in a coma in a place where time around her doesn’t move but her body age a little in that place and she isn’t quite conscious while in it, i believe she feels like just sleeping.
