This is a pretty bad case because of the swelling. Ideally that hooked section of the nail should be removed, but the swollen part is in the way. If it weren’t an issue, a nipper cutter can be inserted between the nail and skin. The user will need to have the skill to line it up and clip just that protrusion, and the problem would be fixed. For that though, you’d need to be very dexterous with your hands and fingers.
I had this done. waiting for the doctor, getting the anesthetic, and waiting for it to fully lock in took much longer than the actual procedure. all that took probably an hour while the actual pressing flesh aside and snipping took 15 seconds? lol.

my worst fear is to go to doc for my ingrown toenails… but then again i just cut the skin myself and pull em out with pliers so 
Glad I’ve never had a swollen toe, worst I’ve ever had was squashed in an escalator
